At Health 4U Clinics, we are dedicated to providing exceptional women's health care as part of our comprehensive family practice. Our mission is to empower women through accessible and affordable healthcare services tailored to their unique needs.

Dedicated Professional Staff

Our provider, April Tolbert, WHNP-BC, brings over 30 years of combined advanced practice and clinical Registered Nursing experience to our family practice. With a special interest in women's health, she is deeply committed to offering affordable healthcare services to all her patients. April is a proud graduate of the University of Central Arkansas and Texas Woman’s University, and she is also a veteran of the United States Air Force, having served as a Captain in the 82nd Medical Group.

Minor Skin Disorders

At our family practice, we offer comprehensive healthcare services that address various skin conditions including acne, athlete’s foot, eczema, psoriasis, ringworm, rashes, shingles, scabies, warts, and skin tags, all while prioritizing women's health.

Health Screening and Management

At our family practice, we offer essential healthcare services including Diabetes Screening, Cholesterol Screening, Hypertension (Blood Pressure) Screening, and Thyroid Screening, all tailored to support women's health.

What types of health insurance plans does Health4U accept?

We accept several health insurance plans. To ensure coverage, please include health insurance details in the online appointment request. A partial list includes:

Does Health4U participate Healthy Texas Women's Program?

Yes! Patients MUST APPLY and GAIN APPROVAL and an ID #Number BEFORE online scheduling an appointment. Healthy Texas Women and Family Planning Program provides the following Free Women's Health care services:

Birth Control
Clinical Breast Exams
Pregnancy Testing and Counseling
Sexually transmitted infection (STI) testing
Postpartum depression treatment
Immunizations
Smoking
Mental Health
Chronic conditions treated: Hypertension, Diabetes, High cholesterol

Who can apply?

Women 18 to 44 yrs
(Women 15 to 17 yrs with parent or legal guardian)
Low income Texas resident, no health insurance, not pregnant

Does Health4U offer Virtual Appointments?

Yes! Please include the words: "Virtual", "Tele-medicine" or "Video" in the online appointment request so we know its a virtual appointment request. NOTE: Virtual appointments are not on-demand. Our staff will schedule virtual appointment requests in the next available virtual appointment time slot. If the appointment request does not include any of the virtual appointment key words, the appointment is assumed in-clinic and all no-show and late cancellation fees apply.

What if unable to make a reserved appointment?

In-clinic booked, scheduled, or online appointment requests, including same day requests, are committed appointments. All appointments missed or without 24 hour prior notice will be charged a missed appointment fee of $50.00 or $75.00 for missed procedure visit. Please be sure to call and leave voice message, or reply to the automated reminder texts or emails during office hours to cancel your appointment without fee. Communicating appointment cancellation or reschedule during off hours (for example 8:00PM, midnight, 2:00AM, etc.) does not count as providing advance notice.

What if unable to commit to a reserved appointment time?

We understand it is difficult for some patients to commit to a reserved appointment time - We get it. Life is busy. For those patients we offer WALK-IN appointments. Here are tips for WALK-IN appointments:

Do NOT CALL clinic and leave voicemail for WALK-IN - Just WALK-IN
Do NOT submit an Online Appointment Request for WALK-IN - Just WALK-IN
Patients Check-in as WALK-IN, for themselves, in person, at the front desk
Our Front Desk will make every effort to accommodate WALK-INs but may request WALK-IN patients to return later during that day or the following day
There are NO Late Cancellation or No Call-No Show fees for WALK-IN Patients
All financial policies, including insurance and self-pay, are the same for WALK-IN as in reserved appointments.

Remember online appointment requests, including same day requests, are committed appointments. If patients are not sure they can make the appointment and do not want the risk of no-show, late cancellation fees then just WALK-IN

What's the deal? Health 4U never answers the phone

Correct! Health4U leverages technology where ever possible. We have several communications channels to communicate with patients.

Voice Mail Message at 817.759.2273 (CARE). Leave a message and we will reply, with text, email, or portal message within 1 business day.
Patient Online Appointment Request. For new or returning patient appointment requests
Patient Portal Message. We provide our established patients access to our HIPAA complaint Patient Electronic Health Records and messaging platform (eHR) for medical discussions with our staff, lab results, medical records access, and patient account payments
Email and SMS Text Messages. NOT HIPAA compliant. For scheduled appointment changes, billing questions, and general NON-Medical and not HIPAA protected questions.

Our technology communication investments is a great convenience for patients because it allows patients to make appointments, see their health information, and ask questions 24X7. It also enables us to focus our attention on in-clinic patient care during clinic hours.
